clutch problems

well i sent car in to garage to have the clutch done with lots of other bits and bobs, now the mechanic took my old clutch out n said cant see anything wrong with it and put the new one in anyway.
But im still having same problem, as soon as the car gets warm it just slips like mad.
the garage has kept the car in to look at it to sort problem for me, but any ideas what it could be as old clutch did exactly same?

Clutch arm could be bent?

Crank rear oil seal laying a film of engine oil onto the friction plate?

Wrong flywheel?

he said rod in cylinder for clutch wasnt opening enough for some reason. he had to cut 5mm off it.

drove it to london and back at the weekend and to birmingham and back today no problems so far.
