Make sure the trigger wheel is mounted solid as if it moves like a mm the car will misfire. I had this problem when I had one fitted for my DTA S60

check vac pipes mate, mine did this when the vac pipe came off my fuel reg.

Also not sure about scores on the center of the flywheel as I've never seen it off. Just saw the pics after it was skimmed.

Not sure as I didn't take it off just saw the clutch in bits on the floor. But it does sound like that could be the problem. Might invest in a new flywheel and have it done again.
